Monday When a downcast, 54-year-o...Monday When a downcast, 54-year-old hypertensive, diabetic, hypercholesterolemic woman who is of recent origin to me complains of episodic "trouble catching my breath," I of course worry about coronary artery disease. still my thoughts soon veer facing in other directions--first, when she demonstrates an episode with unmatched hiccupy inhalations and asks about "panic attacks"; nearest when she'd swallowed her oral hypoglycemic agent unless skipped breakfast; and finally, when she rehearses me about the "angels." Angels? Her face brightens. "I diocese them every day." Huh? What do they contemplate like? "Just like people." nation she knows? Like family members who have died? She laughs, as if I were being silly. "Oh no, they're not my family," she accords She turns wistful. "I wish they'd communicate, if it were not that they won't say anything. They just apply the mind at me." Are they...? I start. "Oh yes" she says emphatically. "They're real, and I wouldn't want to make no use of them. They're like my twins. I just wish they'd talk to me."My piece of work is seldom dull, I ruminate duly recording "sees angels each day" on her medical record point to be solved [i]or[/i] settled list, all the while wondering: are these angels a vexed question at all? "Fax me an order, and I'll descry him tomorrow," a visiting succor says. Clutching a fistful of prescriptions and the promise of a female caller, Mr Jone contemplates happier--and I feel calmer. "I know I should take better care of myself," he says, "but sometimes I ne you to remind me" My heart waste aways How can I stay impatient with this man? Thursday I'm impressed when I read about physicians routinely bringing online sources of evidence-based medicine to the point of care. thus today, when 80-year-old Mrs. Garcia arrives complaining of episodic right hand pain and numbnes I'm determined to tread on the heels of suit. My nice feeling lasts until evening, when I find myself holding the July 15 2003 issue of American Family Physician. There in succession the cover, large as life: "Management of Carpal subterranean passage Syndrome," with evidence galore, and no doubt the source of my vague recollection.
