The Agency for Healthcare Research and Quality (AHRQ) has started a recent monthly peer-reviewed, Web-based medical journal that showcases patient safety reproofs drawn from actual cases of medical errors. The journal courts to educate health care professionals about medical errors in a blame-free environment.
